4. Has anyone living in your household ever had a problem with dogs?

5. Does anyone in your household suffer from allergies, asthma or have any other problem which might be aggravated by dog hair or dander?

7. If you rent, will your landlord be willing to sign a statement allowing you to keep a dog in the apartment/house?

11. How much time do you plan to spend with the puppy?

12. What are you looking for in a puppy? i.e. companion, show, obedience

13. Are you interested in a puppy of a particular sex? Are you seeking a puppy of a particular colour?
